How do bans work?

When players get reported, they're probably looked at. As we could all see there were 500+ peopel banned recently. Now I'm wondering how these bans work. Mainly when you ban an account it's ip based. However I think everyone here knows ip's are about the lousiest way to ban players. Let's suppose this:

Player X has a quitrate of 0.50. Player X also is known for leaving, queue dodging, flaming, raging, feeding on purpose, eating penguins with a spoon,...
Now RIOT says " ok dude (or girl), it's been enough , bwe're banning you. "

So I got the following questions:

1) Is a ban always a permban?

2) When it's a permban, what stops players from returning? Do you guys ban on ip solely or on MAC Hash or ... ?

3) When a player is banned and does return with a new account, assuming he has not switched ethernet cards, pc, provider, country etc etc... will he get banned again?

4) As we know it's quite easy to dodge bans (cfr prpxies, new ethernet card, new provider,..... is there a team working on retracing banned players?

When HGG was banned i believe he could still play on his smurfs. So it is probably just account bans.
I doubt they would immedietly ban a banned persons new account if they didn't just prevent him/her from playing at all initially.

Most bans i hear about are bans that are phrased like they will no longer be banned in 10+ years .

It's nothing to do with IP based, only forum bans can include IP bans, though not all the time.

Ingame bans are account based only.
